Welcome to the MCS Vendor Application

To apply to become a vendor for MCS, you will need to complete the following steps. The document names listed in red are links to PDF files that will need to be included with your application.  ALL OWNERS OF THE BUSINESS MUST SIGN ALL DOCUMENTATION. 

1.      Complete, read and sign the MCS Vendor Agreement.

2.      Complete and sign the W-9 Form.

3.      MCS requires a background check be completed on the owner(s) of the business.  MCS requires all vendor owners complete a background check to ensure that no person who has been convicted of any criminal offense involving dishonesty, a breach of trust or money laundering or who has participated in a pre-trial diversion with respect to such an offense or who has been convicted of a felony or misdemeanor work or perform services on behalf of MCS.  Please complete and return the attached Application Pre-Authorization form.  Based upon need, a Regional Vendor Manager will provide you with instructions on completing and providing this background check to MCS.  PLEASE DO NOT COMPLETE A BACKGROUND CHECK AT THIS TIME.

4.      Send a copy of your current insurance to insurance@mcsnow.com.  To become a MCS vendor you must contact your insurance agent and request an Errors & Omissions Insurance Policy with $1,000,000 coverage limit; MCS must be named as a certificate holder.  No policy changes should occur until you have been approved as a vendor. 

5.      Send a copy of your current insurance to insurance@mcsnow.com.  To become a MCS vendor you must contact your insurance agent and request a General Liability Insurance Policy with $1,000,000 coverage limit; MCS must be named as a certificate holder.  No policy changes should occur until you have been approved as a vendor. 

6.      Complete and sign the Workers' Compensation Form.  Include the necessary documentation when you submit the form. 

7.      Provide a copy of your current Business License.

8.      Complete the Vendor Territory List.

Once you have all the pertinent documents, fill out the application on the following pages.  You may either upload the documents and submit them with your online application or fax them once you have completed the application to (813) 402-1746.  All documents must be signed and dated by the owner(s) of the company.
